Guidance for the New HMG-CoA Reductase Inhibitor (Statin) Contract

Effective Date:  1 May 2003 (Contract will be in effect for one year with an option to extend the terms of the contract for 4 additional one-year periods).

Selected Product:  Simvastatin  (Zocor®) 5 mg, 10 mg, 20 mg, 40 mg and 80 mg tablets; Merck & Co., Inc.

MTF Formulary Guidance

Table 1: Formulary Status of Statins at Military Treatment Facilities

MTFs must have on formulary

MTFs cannot have on formulary

MTFs may have on formulary

All strengths of simvastatin
(5 mg, 10 mg, 20 mg, 40 mg, & 80 mg tablets)

Any strength of:

  • Atorvastatin (Lipitor)
  • Rosuvastatin (Crestor)
  • Ezetimibe/simvastatin (Vytorin)
  •  Branded lovastatin products including:
    • Mevacor
    • Advicor (lovastatin / niacin
      extended-release)
    • Altoprev (lovastatin extended-release)

Any statin or statin combination product that is approved by the FDA after the date the contract was awarded cannot be added to MTF formularies while this contract is in effect.

Generic lovastatin - joint VA/DoD contract for EON Lab’s product (Contract #V797P-9084)

A non-CYP-3A4 statin:

  •  Fluvastatin (Lescol, Lescol XL)

    OR

  •  Pravastatin (Pravachol)

    BUT NOT BOTH

Treat to Goal Chart 

The PEC has developed a chart to assist prescribers to 'Treat to Goal' by matching statin doses and baseline LDL levels. The chart 'does the math' by identifying the best starting statin dose given a baseline LDL and the patient's LDL goal. It also contains additional information which may be helpful: descriptions of CHD risk factors and patient risk groups, common prescribing information, and typical non-formulary statin request information.
